Information required for Blue Badge Application Paper Form/Online

Photo id: Passport/ driving licence.

Proof of address: Council tax bill/Utility bill or bank statement within last 3 months

Photograph: A recent photograph

Medication: List of all up to date medication.

Medical Info: All up to date medical information – Encounter reports can be supplied

Award letters for PIP (Personal Independent Payment) along with points part of the award

DLA (Disability Living Allowance),

AA (Attendance Allowance),

National Insurance number.

Contact Info: Up to date Email address.

Payment will be taken once the Blue Badge has been approved
